Abstract :
Spiritual caregivers recognize it is important to document their interactions with those to whom they provide spiritual care and counsel. The form in which these notes are written is likely to be shaped by the documentation practices of the workplace. This paper seeks to move beyond precept, e.g., 'notes are required', with a view to reflecting on two aspects of the clinical note: the form in which the note is written and the inclusion of spiritually focused interventions in clinical notes. By way of illustration, this paper explores the use of three approaches (SOAP, RIOAP, SSOAP) both to write clinical notes as well as to identify and track medical, psychotherapeutic and spiritual information and interventions. Next, this paper evaluates SOAP, RIOAP and SSOAP, noting the relative strengths and shortcomings of each format in the work of spiritual care and counselling.



Les conseillers spirituels reconnaissent l’importance de documenter leurs interactions avec ceux à qui ils fournissent des soins et conseils spirituels. La forme sous laquelle ces notes sont écrites est susceptible d’être façonnée par les pratiques de documentation du lieu de travail. Cet article cherche à aller au-delà du précepte, par exemple, que «les notes sont exigées», en vue de réfléchir sur deux aspects relatifs au dossier clinique: la forme écrite des notes et l’inclusion d’interventions, spirituellement axées, dans les notes cliniques. Afin d’illustrer ces points, cet article explore trois approches utiles pour écrire les dossiers cliniques (SOAP, RIOAP, SSOAP) ainsi que pour identifier l’information et les interventions médicales, psychothérapeutiques et spirituelles. Finalement, cet article évalue ces approches, SOAP, RIOAP et SSOAP en identifiant les points forts ainsi que les limites de chacune dans le cadre du travail des soins spirituels et du counseling.
