Authors are requested to make their contribution anonymous before submitting, omitting first person references to other material, such as 'see my article' or 'which I published in 2009'. All articles are peer reviewed by two external referees in a double-blind anonymous system. After confirmation of receipt, authors will receive two reports within six weeks.
