A 52-year-old man with anorexia, weight loss and a cough
The medical history of a 52-year-old male patient with obstructive pulmonary disease (COPD), ethyl alcohol abuse, anorexia, a chronic cough and weight loss (8 kg since seven months) is reported. A CT scan of the chest reveals bilateral cavitary laesions, as well as mediastinal and hilar adenopathies. Acid-fast bacilli are stained after a bronchoscopy. Polymerase chain reaction (PCR) indicates an infection with Mycobacterium avium complex, which is treated with azithromycin, rifampicin and ethambutol, with an excellent clinical, biochemical and radiological evolution. One year after the diagnosis, the medication can be ceased, with a good recovery of the patient.
