A 32-year-old man with a painful red ankle
We report the clinical history of a male patient with a non-traumatic, swollen, painful, red ankle and wrists. He reported some pain in the lower thoracic region since a week as well as cough and dyspnoea, but no fever or chills. The pulmonary chest X-ray revealed a bilateral hilar lymphadenopathy.
A periarticular ankle inflammation combined with a bilateral hilar lymphadenopathy is a typical presentation of Löfgren’s syndrome in men. This syndrome is an acute selflimiting form of sarcoidosis. It is important to differentiate both as there is a difference in clinical presentation, therapy and prognosis between sarcoidosis and Löfgren’s syndrome.
