Churg-Strauss syndrome in a 64-year-old man
A case of a 64-year-old man with dyspnoea, fever, loss of strength in both legs, petechiae and gastro-intestinal bleeding was presented. Clinical examination revealed severe wheezing as well as purpura on legs and arms. Laboratory analysis displayed hypereosinophilia, elevated IgE levels and ANCA positivity of the anti-myeloperoxidase subtype. The diagnosis of Churg-Strauss syndrome was confirmed by skin and muscle biopsy disclosing an eosinophilic vasculitis of the small vessels. Our patient improved significantly by glucocorticoid treatment.
The Churg-Strauss syndrome is a rare form of small-vessel vasculitis which presents itself most often with asthma, vasculitis and peripheral neuropathy. Less frequently the kidneys, heart and gastro-intestinal tract may be involved. Almost all patients display hypereosinophilia; 40% have ANCA positivity. Diagnosis is confirmed microscopically on biopsies of the affected tissue. Most patients improve with glucocorticoid treatment.
