A 34-year-old patient with a high fever, a cough and diarrhea
A 34-year-old man presents to the emergency room with complaints of a fever up to 40°C and a non-productive cough with right-sided pleuritic chest pain since one week. He also reports a headache and watery diarrhea. A blood test reveals signs of inflammation, along with a remarkable hyponatremia and elevated creatine kinase and transaminase levels. On a chest X-ray, a right lower lobe consolidation is noted.
Legionellosis is subsequently diagnosed by rapid urinary antigen testing and later on confirmed by detection in a sputum specimen (culture and polymerase chain reaction (PCR)). This case report illustrates how clinical and laboratory findings may be helpful to suggest an etiologic diagnosis in case of pneumonia.
