A middle aged man presenting with sciatica
A 41-year-old man presents with lower back pain radiating towards the left leg. CT scan and MRI reveal an osteolytic lesion in the sacrum. Blood and urine test indicate monoclonal light lambda chains and biopsy of the lesion shows a population of monoclonal plasma cells. X-rays of the axial skeleton reveal no secondary lesions and bone marrow examination shows less than 1% plasma cells, suggesting a solitary plasmacytoma. Solitary plasmacytomas compose 5% of all plasma cell tumors, and are usually treated with radiotherapy.
