A man with stomach complaints, chest pain and shortness of breath
In a 69-year-old man with chest pain, increased shortness of breath and stomach complaints the diagnosis of pneumothorax and pneumonia with empyema was put forwards. Pleural cultures showed Enterobacter species, Proteus species and Eschericia coli. Further investigation revealed gastro-pleural fistula formation provoked by an invasive growing gastric tumor.
In case of the simultaneous occurrence of pneumothorax and empyema, gastro-pleural fistula formation should be considered, especially when bacteria which do normally occur in the gastro intestinal tract, are present.
