Facial edema... not always an allergic reaction!
A 17-year-old girl was referred to the emergency department with complaints of facial edema and dyspnea. A plain chest film of the thorax revealed a large mediastinal mass. Because of vision problems a CT-scan of the brain was executed which showed hydrocephaly. After completion of the examinations with cervical mediastinoscopy, PET scanning and bone marrow examination, the diagnosis of a large B-cell lymphoma was made. The patient was treated with chemotherapy (R-CHOP).
We review the diagnostic approach and initial treatment of a superior vena cava syndrome.
