Abstract :
We present a case of bundle-branch reentry ventricular tachycardia with RBBB morphology (reverse BBRVT) in a patient with preexisting right bundle-branch block and without apparent structural heart disease. The diagnosis of BBRVT may be very challenging due to the similar wide QRS morphology during tachycardia and in sinus rhythm, rather suggesting a supraventricular origin. BBRVT should be suspected even in the absence of structural heart disease, but especially in patients with extensive underlying His-Purkinje conduction disorders. The aetiology, mechanism, diagnostic issues and treatment are briefly reviewed.
