De laatste jaren is duidelijk geworden dat vroegtijdige behandeling met "disease modifying antirheumatic drugs" (DMARD’s) aangewezen is. Rekening houdend met de kostprijs en de bijwerkingen van deze behandeling is deze enkel gerechtvaardigd indien de diagnose met zekerheid kan gesteld worden. De diagnose steunt voornamelijk op klinische kenmerken en slechts in beperkte mate op serologische ondersteuning. Vandaag beschikt de arts echter naast de reumafactor, die gekenmerkt wordt door een beperkte specificiteit, over een meer specifieke merker voor reumatoïde artritis: antistoffen tegen cyclisch gecitrullineerd peptide (CCPII-test). Antibodies to cyclic citrullinated peptide: a specific laboratory test for rheumatoid arthritis Rheumatoid arthritis is a chronic inflammatory autoimmune rheumatic disease with a prevalence of 0.7%. Modern treatment of rheumatoid arthritis is shifting toward an aggressive antirheumatic therapy early in the disease course. Since treatment with disease modifying antirheumatic drugs is only justified when the cost-benefit ratio is favourable, it is mandatory to be able to differentiate between rheumatoid arthritis and other forms of arthritis early after symptom onset. Diagnosis is primarily based on clinical manifestations and serological support has, up to now, been restricted to the determination of the rheumatoid factor, which has a low specificity. Recently, however, a more specific serologic test for RA has been developed: antibodies to cyclic citrullinated peptide (anti-CCPII). |
